Why do patients from Ahilyanagar travel to Pune for fistula surgery?
Mostly for complex or recurrent fistula. A fistula that has come back after one or two operations needs an MRI map of every branch and a surgeon who treats fistula daily. Travelling to Pune lets you do that in one planned trip, with the same surgeon at every follow-up.
Simple piles and many fissures can be treated well close to home, and I say so when that is the case. Where the trip is worth it is a fistula that keeps discharging, a tract close to the sphincter muscle, or piles that have already been operated on once. Those cases need the plan to be right the first time.
The district is also home. My native village is Kolhar, on the Ahilyanagar–Manmad road near Shirdi, and I was born in Shrirampur. In Kolhar, half the village shares my surname. How does a one-trip consultation and surgery work?

Which conditions are treated?
Anal fistula, piles, fissure, pilonidal sinus and hernia. For fistula, the operation — FiLaC laser, VAAFT, LIFT, seton or fistulotomy — is chosen after imaging (fistula treatment in Pune). For piles, early grades are often treated without surgery by rubber band ligation; larger piles by laser or stapler surgery (piles treatment). Also: fissure, pilonidal sinus and hernia.

Does every fistula patient from Ahilyanagar need an MRI first?
Almost every one. I image almost every fistula before I operate — MRI fistulogram or trans-rectal ultrasound — because the tract's depth, branches and relation to the sphincter decide which operation is safe. For outstation patients this helps: the MRI can be done in Ahilyanagar and sent on WhatsApp, so the plan is ready before you travel.
